Bruce Baldwin

Bruce joined Origin Energy in 2010 and has worked within the LNG Business leading the Emergency Management team since 2011. In 2009 he finished 20 years service with the Australian Army as a Lieutenant Colonel commanding the Officer Training Wing and as Chief of Staff, Land Warfare Centre. His time in the military saw highlights of service in Africa and Middle East as well as various command positions within Armoured Units.

Bruce holds Masters Degrees in Emergency Management (CSU) and Defence Studies (UCAN) as well as Graduate Diploma’s in OHS (QUT) and Personnel Management. In 2005 he graduated from the Australian Command and Staff College, a tri-service, one year leadership development course.


Lorena Bueno

Lorena Bueno Cortez currently works for Schlumberger as Health, Safety and Environment (HSE) Manager for Australia Land, based in Brisbane.

Lorena began her career as a field HSE specialist for Schlumberger in Lago Agrio, Ecuador in 2001. Following multiple assignments in Ecuador, Lorena transferred to the USA in 2005 to manage HSE for the Waddell Project. In 2006 she was named HSE Manager for Integrated Project Management (IPM) North America, where among other duties she was responsible for all aspects of contractor safety management. Moving to Malaysia in 2008, she assumed the position of HSE Manager for IPM Malaysia, after which she transferred to Vietnam as Environmental Manager for Middle East and Asia. In 2012, Lorena moved to Brisbane, Australia as Training, Regulatory Compliance and Environmental Manager. Lorena began her current role in January 2014.


Ian Cameron
Head of Chemical Engineering, Faculty of Engineering, Architecture and Information Technology

Ian Cameron is a professor at the School of Chemical Engineering, an inaugural Senior Fellow of the Australian Learning and Teaching Council and ALTC Discipline Scholar in Engineering & ICT. He is also a director and principal consultant at Daesim Technologies, Brisbane. He is a Fellow of the Australian Academy of Technological Sciences and Engineering (ATSE).

He completed Chemical Engineering degrees at the University of NSW, and a masters degree at the University of Washington. He worked for 10 years for the CSR Group in diverse industry sectors such as sugar, building materials and industrial chemicals, having roles in process and control system design, plant commissioning, production management and environmental protection.

He obtained his PhD and DIC from Imperial College London in the area of Process Systems Engineering (PSE), and then worked full-time for 3 years as a United Nations (UNIDO) process engineering consultant in Argentina and a further 6 years in Turkey on a part-time basis. He has spent the last 25 years in research, consulting, teaching and learning innovation at The University of Queensland, having received numerous awards including the J.A. Brodie Medal of the Institution of Engineers Australia, the Australian Award for University Teaching in Physical Sciences 2003 and the Prime Minister’s Award for University Teacher of the Year. He was part of the team from UQ Chemical Engineering that won a national AAUT institutional award in 2005 for educational enhancement via project centred curriculum and course innovation.


Professor Sidney Dekker
Griffith University, Brisbane

Founder, Safety Science Innovation Lab

Sidney has gained worldwide acclaim for his groundbreaking work on human error and safety. He was previously at Lund University in Sweden as Professor where he founded the Leonardo da Vinci Laboratory for Complexity and Systems Thinking, as well as an MSc in Human Factors and System Safety. He has also been a Senior Fellow at Nanyang Technological University in Singapore, and Honorary Professor in the Department of Epidemiology and Preventive Medicine, Monash University in Melbourne, Australia. He has also been visiting Professor of Community Health Science at the Faculty of Medicine, University of Manitoba, in Canada.


Jason Edwards

Dr Jason Edwards is a research associate and research ethics advisor with the Centre for Accident Research and Road Safety – Queensland (CARRS-Q) at Queensland University of Technology. With an academic background is in Psychology, he conducted his PhD in Organisational Psychology and Safety. This PhD project consisted of a three year investigation of the culture of the Australian Heavy Vehicle Industry and the influence this culture has on safety. Dr Edwards has been involved in a number of other research projects both in Australia and in Oman, and has presented at domestic and international conferences. In addition to his research roles, Dr Edwards supervises PhD students, provides ethics advice to researchers in CARRS-Q and has served as a peer-reviewer on a number of well-respected scientific journals. . Jason has a particular interest in applied research which results in real world application and change, and in communicating research findings to non-researchers. His research areas have included reviews of policing policy and practice, examination of heavy vehicle safety and safety culture, traffic safety culture and investigating effective practices for workplace health and safety inspectors.  Dr Edwards has developed a keen interest in effectively communicating safety to result in behavioural change, as well as the influence that culture has on communication of safety.


Noyan Ercan

Noyan Ercan, CPEng (Mech) is the Lead Asset Integrity engineer of Arrow Energy in Brisbane.  Noyan completed his Bachelor of Mechanical Engineering degree in 1993 and started his career in mechanical design and manufacturing in Turkey. Shortly after completing his Masters degree in Asset Management and Reliability Engineering at the Newcastle University, he joined Shell Refining in 1998 as a reliability engineer and held a number of different operational, maintenance and project roles within Shell.

Noyan completed a Master of Engineering Management degree at UTS in 2006 and straight after joined Shell Global Solutions at the Shell’s central headquarters as a Senior Asset Management Consultant and Program Manager. He led several integrated Business Improvement Programs and Technical Service Agreements at Shell Partner Refining Companies in the MENA region with program scopes including HSE, Asset Management, Energy Road Map Development and Technical Competency Management. Noyan’s professional mastery areas are maintenance & reliability management, root cause analysis, risk based inspection, Asset Integrity and Process Safety Management.


Dr Jan Hayes

Member, Advisory Board of the National Offshore Petroleum Safety and Environmental Management Authority, NOPSEMA

Dr Hayes has 25 years’ experience in safety and risk management. Her experience covers a range of industries including offshore oil and gas, onshore petrochemicals, gas pipelines, mining, manufacturing and transportation, and air traffic control. She began her career in 1983 in oil and gas process plant operations, and then moved into detailed design. She has been involved in the initial Australian oil industry response to the Piper Alpha incident in 1998 and has worked almost exclusively in the field of industrial safety since then.

Between 1991 and 2004, Dr Hayes was a director and part owner of Qest Consulting Pty Ltd, one of Australia’s best known industrial risk and safety consultancies. Holding a number of operational roles, she managed up to 50 staff and was personally responsible for projects ranging from technical safety studies such as: safety cases, HAZOP studies, quantified risk assessments and reliability studies, to organisational psychology reviews and human factors studies. She now holds a Senior Research Fellow appointment at the Australian National University where she is Program Leader for research on public safety for the Energy Pipelines Co-operative Research Centre. Her research interests include operational decision making, safety in design, engineering professionalism, use of standards and safety indicators.

Gavin Holden

Gavin has over 20 years of operational and management experience with an Australian Fire & Emergency Service,  As the Manager of Professional Development he is responsible for the implementation and support of career progression and skill development of part time and full time fire and emergency staff required to meet the needs of an all hazards emergency services organisation.

He has worked in the Middle East with a leading Liquid Natural Gas producer, stewarding the company’s Centre for Safety Training implementing and ensuring SHE&Q training and compliance with job competencies. Currently he is leading a multi-national work team; implementing training standards, delivery procedures, record keeping, incident investigation and quality assurance, using strategic management, change management and leadership skills.


Dr. Clive Killick

Dr Clive Killick has more than thirty years’ experience of process, safety engineering, and HSE consultancy in hydrocarbon gathering / processing facilities and air separation technology, most recently at Royal Dutch Shell and in his current role at Arrow Energy.

He has authored several Shell HSE documents including the Shell Contractor HSE Greenbanding Process and the guidance document HSE Assurance for Capital Projects (Project Guide 1) which is used as the template for determining the number and scope of HSE activities in a project. Clive is also the author and presenter of a course on risk management at the University of Queensland and, in 2014, was awarded the title of Adjunct Professor in Chemical Engineering.
In his spare time Clive is a rower, coach and umpire. He was the British Rowing Honorary Safety Advisor and Chairman of the National Safety Committee (2010-2013), and a member of the safety team for the Olympic and Paralympic Games in 2012.

Kevin joined Talisman Energy as the Senior Vice President of Drilling and Completions in February 2010. Kevin Lacy has worked in the oil & gas industry for the past 32 years. His career began with Chevron U.S.A. in New Orleans, Louisiana. Kevin rose through the ranks of Chevron in various roles in drilling, production, and asset management to ultimately become Vice President of Global Drilling and Completions at the merger of Chevron and Texaco.

After spending 26 years with Chevron, Kevin retired from Chevron and joined BP in July 2006 where he initially held the role of D&C Head of Discipline for the Western Hemisphere. He then held the position of VP for Drilling and Completions in the Gulf of Mexico. He was responsible for the central team established in 2008 to manage all drilling and completions operations for the Gulf of Mexico.

Kevin has traveled to over 45 countries and lived in five outside the US. He holds a bachelor of science with honors in Petroleum Engineering from the University of Tulsa and an MBA from the University of California at Berkeley. Kevin received the International Association of Drilling Contractors Exemplary Service Award in 2007. He was elected to the Tulsa University Engineering Hall of Fame in 2002.


Andrew Law

Andrew Law is the General Manager for Weatherford Australasia (Australia and New Zealand).

He holds a bachelor’s degree in mechanical engineering and started his career in the oil and gas industry over 18 years ago as a wireline field engineer. His first role as a field engineer in the North Sea included working on major HPHT projects, then he moved to Africa working on deepwater exploration projects, after which he moved to the HPHT fields of South Texas in a technical role before returning to the North Sea in a sales role.

Following his career in an oil and gas service company, he then moved into a corporate finance role in a major accountancy firm, advising on mergers and acquisitions in the oil and gas service sector.

In 2007 Andrew joined Weatherford Optical Sensing Systems team in a business development role, after which he moved into a business development role based in London. He moved into his current role of General Manager Australasia in March 2011.


Stephen Matheson

Stephen is the Chief Inspector, Petroleum and Gas in the Queensland Mines and Energy group of the Department of Natural Resources and Mines.  The Petroleum and Gas Inspectorate that he heads is responsible for regulating safety of all petroleum and flammable gas related industries and general flammable gas related safety matters in Queensland.

Stephen has been working in the petroleum and gas safety regulation area for nine years and been chief inspector for the last 7 years.  He played major role in developing the safety and coal seam gas aspects of the Petroleum and Gas (Production and Safety) Act 2004 introduced in December 2004.  He has previously worked in various areas of the former Department of Mines and Energy related to regulation, policy development and facilitation and administration of petroleum and gas, coal seam gas, coal and oil shale related projects.

Stephen has produced numerous papers and made presentations on a wide range of energy resource and safety topics.  He represents Queensland on the Gas Technical Regulators Committee and is the current chair of that committee.  He has been active in many steering committees and working groups across a range of national issues.  Stephen has Masters Degrees in both Geology and Public Policy.


Dr Russell Mills

Russell has a background in process design, development and operations based on 15 years chemicals industry experience as an employee of ICI Australia / Orica, where he worked in the development and manufacturing of Ammonium Nitrate explosives for the mining industry.  Since entering the Consulting Industry in 1997 he has gained extensive experience in both qualitative and quantitative risk analysis and assessments in a wide range of industries to assist his clients gain an understanding of their business-critical risks and how to manage them.  Russell currently leads a number of risk-based process safety studies with the coal seam gas industry in Queensland and is an accredited HAZOP Leader.

Russell’s has a Bachelor of Science with Honours majoring in Chemistry from the University of Melbourne and a Ph.D in Engineering Science from Monash University.

Gavin is a chartered chemical engineer with 18 years of experience in many industry sectors including Hydrocarbons, Minerals, Metals and Chemicals.

Gavin is responsible for the functional and technical management of the Safety and Risk Engineering group in Australia East. The WorleyParsons Safety and Risk Engineering group consists of 19 Safety and Risk and Fire Protection Engineers ranging from Graduate to Chief Engineers based in Brisbane and Melbourne.

Prior to joining WorleyParsons Gavin worked with Origin Energy as a Process Safety Manager and ExxonMobil in both operations and process safety/risk management.

Gavin is currently supervising a QRA for the Hebron offshore facility for ExxonMobil Canada, Process Safety Studies for a new gas plant construction in Alaska, the design of all fire protection systems for two electricity plants in Saudi Arabia and the Nyrstar Transformation project, a risk management study for the Victorian Government and numerous process safety studies for QCG and Santos.

Rowena Richardson

Rowena Richardson is the Director, Emergency Management Standards, Best Practice and Evaluation with the newly formed Queensland Office of the Inspector-General Emergency Management. In this role, Rowena is leading the development and implementation of an Emergency Management Assurance Framework and the application of review and evaluation methodology within the Queensland Emergency Management sector.

Prior to this role, Rowena was a Director with the former Emergency Management Queensland with the responsibility for disaster management doctrine, planning and the assessment and improvement strategies for Queensland’s Disaster Management Arrangements. She led disaster management programs based on a risk management approach and evidencebased practice to improve the effectiveness of disaster management across Queensland. Preceding working within the emergency management sector, Rowena had extensive career in health care; as a critical care clinician, educator, researcher and policy maker. In 2005 she formed part of the senior management team in Queensland Health’s Patient Safety and Quality Improvement Service leading state wide patient safety programs. She posses’ specific expertise in: policy development and implementation, planning, change management, safety management systems, human factors science, team communication, systems analysis and improvement and performance assessment.

Stuart Smith

The National Offshore Petroleum Safety and Environmental Management Authority, NOPSEMA

Stuart Smith was appointed Chief Executive Officer of the National Offshore Petroleum Safety and Environmental Management Authority (NOPSEMA) in September 2014; Australia’s independent regulator of offshore safety, well integrity and environmental management of offshore oil and gas facilities around Australia.

His appointment followed six years as Director General of the Department of Fisheries (DoF) in Western Australia where he managed the sustainable use of aquatic resources in West Australian and adjacent Commonwealth waters. These responsibilities included developing and regulating commercial fishing, aquaculture, recreational fishing, aquatic biosecurity and selected aquatic safety and marine park compliance functions.

Stuart also worked as a Deputy Director General and Acting Director General with the Department of Industry and Resources (DoIR) in Western Australia. As Deputy Director General for State Development he was responsible for facilitating major mining and oil and gas projects, attracting investment, and promoting trade and economic expansion opportunities in WA.  As Deputy Director General for Resources he was responsible for promoting resources exploration in the state, regulating mining activity, managing resources sector royalties and regulating oil and gas activity including onshore and offshore safety and environmental management.

Prior to joining DoIR in 2003, Stuart spent 14 years with the Australian Public Service in Canberra, Melbourne and Perth. During this period he held various industry development and regulatory roles including positions with the Industry portfolio and the Australian Competition and Consumer Commission, together with a secondment to Parliament House as an Inquiry Secretary and advisor.

Stuart holds a bachelor of economics degree from the University of Western Australia and a Graduate Diploma in Economics from the Australian National University.


Toby Stone

Toby Stone is the General Manager of the Marine Environment Division. His role is to manage Australia’s National Plan for Maritime Environmental Emergencies, regulations to minimise the risk of pollution from ships, and the National Emergency Towage Program.


Ian Travers

Health and Safety Executive (HSE)

Ian is head of Chemical Industries Strategy Unit in HSE with responsibility for setting the direction and operational policy for the regulation of chemical industries in the UK. He was responsible for managing HSE’s strategic response to Buncefield and steered the Process Safety Leadership Group to conclude a joint regulator and industry response to the incident and other major incidents such as BP Texas City. From this experience, Ian was promoted for the UK chemical industry to develop key principles of Process Safety Leadership.

He shaped recent changes to the way the HSE and the UK Environment Agencies jointly regulate major hazards within the COMAH remodelling program, resulting in improved efficiency in the regulation of major hazards and regulatory programs. He also pioneered the use of key performance indicators to monitor the effectiveness of critical chemical process systems. Ian was the first author to present this in ‘Developing Key Performance Indicator for Process Safety, HSG 254. He also worked as a drafting author on the OECD guidelines for KPIs and contributed to the CCPS and KPI guidelines.

Ian recently chaired the international expert panel to publish the OECD guidelines on Process Safety Governance, and he worked with ScottishPower in the UK to develop their award winning approach to asset management.


Chris Warnock

Chris is Queensland Division President of Engineers Australia for 2015 and is passionate about representing the interests of the engineering profession and the members of Engineers Australia. Chris is a Fellow of Engineers Australia, and a Chartered Professional Engineer.

Chris has been a Civil Engineer for 40 years, with a working history predominantly in the rail and transport industry, both within the public and private sectors, having held a variety of senior roles across design, construction, strategy, operations, and business management.


Andrew Wilson

Throughout his career Andrew’s expertise has had a focus on operational and business excellence, including ensuring an injury free workplace. He has specialised in providing consulting solutions to organisations in the areas of Operational Excellence, Contractor Safety Management and Process Safety & Risk Management (PSRM).

Andrew has led significant change projects that have delivered improved plant performance as well high performance cultures. This has been achieved both by improving the reliability of equipment, as well as engaging with the workforce to understand and remove barriers to best practices. Andrew has led cultural change programs in highly unionised workforces that have resulted in zero injury workplaces, together with improved productivity, reduced absenteeism and harmonious industrial relations.

Andrew holds a Master of Business Administration (MBA) from the University of Melbourne and a Bachelor of Engineering majoring in Mechanical Engineering from the University of Western Australia.

Nathan is a Charter Professional Member and Director for the Safety Institute of Australia, the Chair of the Finance, Audit and Compliance Committee of the Safety Institute of Australia and a member of the WA Chamber of Commerce and Industry OHS&WC Committee.


Rhonda Yoder

Rhonda Yoder is the general manager of Operational Excellence (OE) & Health, Environment and Safety (HES) for Chevron Australia located in Perth, Western Australia.

Prior to her appointment in August 2013, Rhonda held the position of general manager for OE, HES & Security for the Asia South Business Unit in Bangkok, Thailand. The Chevron Asia South Business Unit is responsible for interests and activities in Bangladesh, China, Thailand and Cambodia / Vietnam.

Rhonda joined the Chevron Corporation in 2003 and held various managerial positions in OE and HES with the Downstream, Project Resources Company and the Upstream Business around the world. Managerial positions include manager of OE & HES for the Chevron Project Resources Company in Houston, manager of OE for Chevron Global Supply & Trading and manager of HES & Security for the Chevron Pascagoula Refinery.

A chemist by training, Rhonda has worked within the HES function since the early days of her career. Rhonda graduated in 1994 from Illinois Institute of Technology in Chicago with a Master of Science in Environmental Engineering.
